Start
Fish Hoek Bay
Alternative is to go around Miller's Point and come in at the south ramp, next to the Fishing Club.
But if the ocean swell is running from SE, it can get gnarly, coming into Rumbly Bay and onto the ramp - as in you have to negotiate a sizeable shore break onto a concrete ramp!

Weather & more
This run is usually done in winter; cold fronts coming past Cape Town produce NW winds.
Don't do this run in strong (i.e. >25kt if the wind is west of NW).
Warning: On occasion, the wind can rebound off the mountain to blow directly off shore 2km from the finish. This can be very uncomfortable.
Because this run starts flat with an offshore wind, it's regarded as being the easier direction of the Miller's Run.
But it's actually more dangerous than a regular Miller's simply because the wind will take you offshore.
If you can't remount, for example, you'll be blown out of False Bay and towards the South Atlantic and you'll die.
But usually, it's a great run - flat to begin with, but the runs start shortly after you leave Fish Hoek (or, if the wind is strong enough, before you even leave Fish Hoek!) and get better and better.
There's often a bit of side-on wind just before the lighthouse, so you need to be comfortable with working right, surfing diagonally on the runs.
After the lighthouse, it's often delightful - clean runs that you can link forever.
